Pericardial mesothelioma affects the pericardium which is the membrane made up of mesothelial cells that surrounds and protects the heart. This membrane has two layers:
A} an outer layer called parietal pericardium
B} an inner layer called the visceral pericardium {epicardium}

Cause

Unlike in the case of pleural mesothelioma, there is no clear link between this cancer and exposure to asbestos fibers, but a number of patients with pericardial mesothelioma have a history of exposure to asbestos.

Some schools of thought believe that it is the inhaled asbestos fibers in the lungs that travel via blood from the lungs to the heart, although this theory is not supported by conclusive evidence.

When asbestos fibers get lodged in the pericardium, the body is unable to excrete the asbestos fibers and they remain stuck in the pericardium. These fibers over a long period of time cause the pericardial cells to undergo changes that result into cancerous transformation.

The cancer cells divide without control and they eventually lead to the thickening of the pericardial membranes eventually causing development of the tumors. These changes lead to the accumulation of fluids in between the two layers which combined with the thickening of these layers result in the compression of the heart.

Symptoms

Most of the symptoms are caused by the accumulation of fluids and the thickening of the pericardial layer. The common symptoms are:
A) Heart palpitations or irregular pulse (arrhythmia)
B) Chest pain
C) Murmurs (abnormal heart sounds)
D) Cough
E) Dyspnoea (shortness of breath)
F) Fatigue
H) Fever or night sweats
I) Orthopnoea (breathlessness on lying down)

These symptoms are not specific for the cancer, making the clinical diagnosis very difficult.

Diagnosis

After getting a clinical diagnosis a series of tests can be done to locate the cancer and identify the cancer cells.

-MRI, Magnetic Resonance Imaging

This is useful in assessing and determining the extent of development of the cancer.

-Biopsy

This involves the removal of pericardium tissue or fluid for laboratory test to confirm the presence of mesothelioma cells.

-Chest x-ray

This helps to identify the tumor, there is a characteristic shape of the heart on the x-ray film when there is a tumor of the pericardium.

-Chest CT scan

This also helps to identify the tumor.

Treatment

Pericardial mesothelioma patients are not usually suitable for surgery, except in rare cases where the cancer is diagnosed very early and surgery may be done to remove small localized tumors.

This type of surgery is very risky because of the proximity of the heart.

Palliative treatment is often the only option for treating pericardial mesothelioma, this type of treatment is aimed at improving the patient’s quality of life and reducing the severity of symptoms caused by the accumulation of fluid in the pericardium.

Fine needle aspiration may be done to drain the accumulated fluid surrounding the heart. Radiotherapy could also be used to shrink the tumor to alleviate the symptoms caused by the compression of the heart by the accumulating fluid. The proximity of the heart and lungs however makes this a very risky procedure.

Prognosis

The prognosis of this type of mesothelioma is very poor, about 50 -60% of all cases die within 6 months of diagnosis. Occasionally few patients survive for longer periods following treatment with partial surgical resection and radiotherapy.

Insomnia Prevention

If the clot becomes large enough, it can block blood flow, suffocating large patches of cardiac muscle, an event known as a heart attack or angina. Or if a blood vessel to the brain closes off or ruptures, it will result in a stroke. While the exact causes of coronary heart disease are imperfectly understood, certain major risk factors have been identified, including genes, gender, diet, and lifestyle – smoking, exercise, and stress.

Scientific evidence indicates that diet is vital to whether your arteries clog or your heart gives out. Stopping the progression of artery disease in the first place with your diet is foremost in warding off heart attacks and strokes. Even if you already had heart problems, including a heart attack, changing your diet now may prevent future cardiac problems and even halt or reverse arterial damage, helping restore arteries to health.

*Some foods to consider eating more often:

-Sweet potatoes

-Green leafy vegetables

-Carrots, Broccoli and Greens (lightly cooked to keep the carotenoids)

-Pumpkin, canned or cooked

-Squash

-97% or greater fat free chicken or turkey breast (I look for 99% fat free.)

-Low fat tomato sauces and pasta

-Onions and Garlic (Chop or crush to release the photonutrients)

-Homemade pizza with 99% fat free chicken as meat sauce

-Foods with low/no salt for those who have high blood pressure

-Peanuts, walnuts, almonds in moderation (be careful not to gain weight)

-Olive oil and canola oil substituted for other oils, (the key is to monounsaturated fats vs trans-fatty acids or partially hydrogenated fat)

-Salmon and other fish (mackerel, sardines, herring), including the skin and fat (Research suggests this omega 3 fat (EPA fat) has the ability to raise HDLs.

-1-5 servings per week recommended)

-Defatted soy flour (at least 1/3 of a cup per day recommended)

-Fat free milk (skim)

-Oatmeal, shredded wheat, low-no sugar added cereals

-Whole wheat bread

-Fresh fruits

-Apples (with skin for flavonids)

-Oranges (Eat pulpy parts for flavonids)

-Red or black grapes

-Grape juice (1 cup per day recommended)

-Grapefruit, especially pink which has 40% more beta carotene than white

-Dried fruits, especially apricots, dates, prunes

-Cantaloupes

-Fat free homemade yogurt with extra dry milk to increase the magnesium and calcium content

-Tupelo honey as a substitute for sugar in cakes, cookies, breads, etc.

-Salad dressings and dips with non-fat sour cream or homemade yogurt

-Baked whole wheat chips and tortillas

-Bean and chickpea dishes and dips

-Tomato salsas

-Walnuts (for omega3)

-Almonds and Avocados (for monounsaturated fat)

-Broccoli Sprouts

-With all of this you need to maintain your normal weight by watching your calorie intake, and getting plenty of exercise

**Foods to consider avoiding:

-1%, 2% and whole milk

-Meats with 96% or less fat

-Red meats

-Hydrogenated oils such as stick margarine, and when listed as an ingredient in foods

-Food with high butter fat and other animal fats, e.g., cheese, full fat yogurt, sauces

-Hot dogs, hamburgers

-Deep-fried foods

-Sugar

-Ice cream

-Salt (if you have high blood pressure)

-Candy, baked goods and ice cream made with fats

-High fat snacks, chips

-Pies, pastry’s, cookies made with fat and sugar (plenty of healthy cookies, pies, pastry’s in my recipes)

***Foods with anti-heart disease activity can:

-Block platelet aggregation (clotting)
-Reduce blood vessel constriction
-Increase blood flow
-Lower fibrinogen (clotting factor)
-Increase fibrinolytic (clot-dissolving) activity
-Block cell damage from oxygen free radicals
